We support adults aged 18+ with learning disabilities, autism, mental health and complex needs to build confidence, develop independence and achieve what matters most to them.

Founded on a Simple Belief

Momentum Care was founded on a simple belief: every person deserves to be valued, understood and supported to live a life that is meaningful to them.

Exceptional care begins with seeing the person first, not their diagnosis, their challenges or the level of support they require.

Every person has their own story, personality, strengths, preferences and aspirations. Taking the time to understand what matters most to each person is the foundation of everything we do.

Progress Is Personal

Our name reflects our purpose. Momentum represents progress, possibility and the belief that every person's journey is unique.

For some people, progress may mean developing new skills, building confidence or achieving personal goals. For others, it may mean maintaining valued routines, strengthening communication, building relationships, accessing the community or experiencing moments of connection, happiness and fulfilment.

We do not measure progress by a single definition of success. Whatever progress means for each person, we believe everyone deserves the right support, opportunities and relationships to live well.

Our mission. Momentum Care exists to create momentum in people's lives. Through personalised, strengths-based support, we work alongside people to understand what matters most to them, promote choice and wellbeing, and create opportunities for meaningful outcomes.

Our Values

Our values are more than words. They guide every decision we make, every relationship we build and every action we take. They define who we are, how we support people and the standards we hold ourselves to every day.

Building the Right Support in Our Communities

The need for high-quality, person-centred community support continues to grow. According to NHS England data, around 1,800 adults with a learning disability and/or autism were in mental health inpatient settings at the end of March 2025, highlighting the continued need for the right support to be available within local communities.

At Momentum Care, we want to be part of the solution. We work in partnership with people, families, local authorities, NHS teams and other professionals to provide responsive, person-centred support that promotes choice, independence, wellbeing and community inclusion.

Whether supporting a planned transition from hospital, helping to prevent a breakdown in existing support, or enabling someone to develop greater independence, our focus is simple: providing the right support, at the right time, built around the person.
